High Blood Pressure, High Cholesterol, or Diabetes

Use the Family Health Center (FHC) if you have one at your distribution center (DC). Meet with a skilled nurse practitioner to design a personalized plan for managing your condition.

Cost: FREE exams all year long with treatment for these three conditions.

Biometric Screening (BMI, Cholesterol, Glucose, & Blood Pressure)

Use the Family Health Center (FHC) if you have one at your distribution center (DC). Qualified staff will perform a screening. You can contact your FHC by calling 217-773-6060 or visiting www.mypremisehealth.com Dot also offers onsite screening events at some locations.  You can also make an appointment with your primary care physician and have the testing done there. 

Cost: FREE annual health evaluation. FREE blood work at an FHC, onsite events, or your physician’s office. Plus, if you meet specific health goals, you’ll earn money towards your Health Savings Account (HSA).

Fever, Congestion, Ear Pain, Sore Throat, Rash, & Many Other Non-emergency Symptoms

If you are experiencing non-emergency symptoms, you can use MDLIVE. Speak to a certified doctor from the comfort of your home and obtain a prescription if needed. 

Cost: Starts at $44 per visit. Payment can be made with your HSA debit card or other charge card.
